Your copay, deductible, and coinsurance are determined by your individual insurance plan. We recommend calling the member services number on the back of your insurance card before your visit to understand your out-of-pocket responsibility.

- Deductible
- The amount you pay out-of-pocket before your insurance begins covering costs.
- Coinsurance
- Your share of costs after your deductible is met, expressed as a percentage.
- Out-of-Pocket Maximum
- The most you'll pay in a plan year — after this, insurance covers 100%.
