Bring Your Insurance Card

Bring your current insurance card and a valid photo ID to every visit.

Bring a complete list of all current medications, supplements, and vitamins — including dosages.

Arrive 15 Minutes Early

New patients should arrive 15 minutes before their scheduled appointment to complete any remaining paperwork.

Be prepared to discuss past surgeries, hospitalizations, chronic conditions, and family health history.
